An eligible athlete must be a senior, be in good academic standing, live in Crookston or the surrounding area and possess the following Choice Therapy Values: teamwork, compassion and integrity.

    Choice Therapy in Crookston is starting an Athlete of the Month program. An eligible athlete must be a senior, be in good academic standing, live in Crookston or the surrounding area and possess the following Choice Therapy Values: Teamwork - flexibility in ability to help and contribute to a team, willingness to help others, ability to utilize and work with all team members to better others and self,  develop and build relationships and engage in community involvement.

    Compassion: concern/caring for others, showing kindness and willingness to help others, having positive interactions with all individuals, desire to serve others and an ability to develop a positive and meaningful relationship with others.

    Integrity: accountability towards actions, trustworthy, dependable, responsible in decision making, honest and ability to follow moral principals.

    Nominations and applications are submitted by teachers, coaches, fans, family or friends. Applications must include: athlete name, phone number, address, birthdate, email, parent(s) names, high school, sports played and how they possess the Choice Therapy Values. Upon notification as a winner, a parent/guardian must sign a waiver acknowledging the release of the winner’s name and photo to social media as well as media outlets for recognition.
